World Situation of Cancer:

There are over 20 million people living with cancer in the world today. The majority lives in the developing world. Cancer is spreading so fast  that each of us will be touched either as a patient, a family member or a friend. Cancer, evoking such desperation that it has become a metaphor for grief and pain, a scourge straining our intellectual and emotional resources. The world still conjures up deep fears of a silent killer that creeps up on us without warning. 

Tobacco has been identified as the single largest preventable cause of cancer in the world today; it is responsible for about 30% of all cancer deaths in developed countries and a rapidly rising proportion in developing countries  in the underprivileged community. It is the only consumer product available which kills half its regular users.

Situation of Cancer in Bangladesh:

 With  the increase of the  incidence of Cancer  in the country, it has become a matter of serious concern for Bangladesh. Although definite figures about the incidence of cancer is not available in Bangladesh,  it is estimated that there are around 800,000 cancer patients in the country and about 200,000 new cases are being added every year. Around 150,000 cancer patients die annually. These figures, according to Bangladesh Cancer Society, translate to:

- A prevalence rate of 7.10 per 1,000 persons
- An incidence rate of 1.8 per 1,000 persons
- A mortality rate of 1.3 per 1,000 persons as compared to an overall crude death rate of 4.8 per 1,000 persons


Against this background, Dhaka Ahsania Mission is proposing the development of MULTI-MODALITY CANCER CARE as a CENTER OF EXCELLENCE within the proposed AHSANIA MISSION CANCER and GENERAL HOSPITAL (AMCGH). Specific services and resources that will be incorporated, include:

- Surgical Oncology
- Medical Oncology
- Radiotherapy
- Brachytherapy
- Other Oncological sub-specialties including, Histopathology, Epidemiology and trained
  Oncology Nurses
- Diagnostic services including Tumor Markers, a comprehensive array of Imaging
   technologies, Endoscopy, Colonoscopy, Bronchoscopy etc
- Support Services including, Pain Management, Clinical Dietetics, Hospice / Home Care,
   Prosthesis, Counseling and Cancer Support Groups

Mission
The primary mission of the proposed AHSANIA MISSION CANCER AND GENERAL HOSPITAL (AMCGH) is care of cancer patients. The incorporation of generalized health care services is prompted by a number of considerations, namely:
- To ensure economies of scale through increased volume of patients
- To capture revenues from the provision of general acute care services to cross-
   subsidize, expensive cancer treatment
- To establish a sufficiently diverse clinical milieu to support Dhaka Ahsania Mission's
   medical education goals

Approach-Planning & Design Consultants
Design Alliance - Health Care Consultants, Architects and Planners, based in Baltimore Maryland was commissioned to assist in development of this project. Design Alliance was established in 1990 by Tarek Alam and Ali Bigdeli. In establishing Design Alliance, they brought with them over forty years of Health Planning, Programming, Design, Hospital Administration and Construction Management experience to the firm.

Major Projects executed and works in progress include:
1. Helix Health Park (Ambulatory Care Complex in Severna Park, Maryland)

2. Harbor Hospital - ICU, Cardiac Cathetertization, Radiology, Emergency, Physical
   Therapy Department and inpatient Oncology Unit modernization

3. Holy Cross Hospital, Silver Spring, Maryland, Additions and Alterations to include
   Post Anesthesia Care Unit, ICU, CCU, Telemetry Unit, Labor and Delivery Suite,
   Neonatal Intensive Care Unit

4. Holy Cross Hospital, Kitchen and Cafeteria modernization in association with Cini
    Grissom and Associates

5. Design of the Institute of Child and Mother Health, Matuail, Dhaka

6. Management Consultant Institute of Child and Mother Health, Matuail, Dhaka, for
   development of a medical records system, patient registration and MIS Protocols,
   quality assurance system, staffing requirements

7. Design of 250 Bed Gopalganj Sadar Hospital

8. Design of 600 Bed, Dhaka Medical College Hospital-2

9. Upgradation of 328 Upazila Health Centers throughout Bangladesh

Co-ordination

In February 2003, a Design Development Committee (DDC) was appointed by the Executive Director, Dhaka Ahsania Mission to advise and provide guidance on the planning and design aspects of the proposed development. The Committee appointed included:

- Engr. Chowdhury Ziaur Rasul, Consultant, DAM - Member, DDC

Several meetings of the Design Development Committee were held to define the SCOPE OF THE PROJECT, REVIEW SITE PLANNING OPTIONS, REVIEW DEPARTMENTAL/FUNCTIONAL ADJACENCIES and CONCEPTUAL DEPARTMENTAL LAYOUTS.

The conceptual planning framework featured in this presentation has evolved from this process.
